1. Acceptance of These Terms

These Terms and Conditions ("Terms") govern your access to and use of the Medicaid Meals website and related services operated by Vista Verde Homes ("we," "us," "our"). By accessing or using the website, completing a screening form, or submitting information, you agree to these Terms. If you do not agree, please do not use the website. If you use the website on behalf of another person, you represent that you are authorized to do so.

2. Nature of the Service

Medicaid Meals is an educational, information, and referral resource only. We provide general (non-clinical) information, eligibility pre-screening, resource navigation, and referrals to public, nonprofit, and community-based organizations that may offer nutrition-support services.

We are not a healthcare provider, meal provider, insurer, health plan, government agency, broker, or agent, and we are not affiliated with or endorsed by any government entity. We do not determine eligibility, enroll individuals in any program, process applications, deliver services, or guarantee any benefit. All eligibility decisions, enrollments, and services are provided solely by independent third-party organizations and authorized agencies.

3. No Guarantee; No Professional Advice

Using the website or completing a screening does not guarantee eligibility, approval, or receipt of any service. Benefits vary by state and program and may be unavailable in your area. Content on the website is for general educational purposes only and is not medical, legal, financial, insurance, or other professional advice, and does not create any professional relationship. 4. Neutrality and Freedom of Choice

We present available options neutrally and do not recommend, prefer, or steer you toward any particular provider. Participation in any program is voluntary, and you retain full freedom of choice among providers and services. We do not accept any payment in exchange for featuring or referring to any provider, and any provider names shown are informational only.

5. Eligibility to Use the Website

The website is intended for individuals seeking information for themselves or for someone they are authorized to assist, and for healthcare professionals, case managers, and care coordinators making referrals. You must be at least 18 years old (or have authority to act for the individual concerned) to submit information.

6. Your Responsibilities

By submitting your contact information, you authorize us and participating organizations to contact you regarding eligibility screening, referral coordination, and program information. Communications may occur by phone, email, postal mail, secure electronic systems, and — where you have provided the legally required prior express written consent — text message (SMS). Message frequency varies; message and data rates may apply. You may withdraw consent or opt out at any time as described in our SMS/TCPA Policy (linked in the footer) (for example, by replying STOP to texts). Consent to receive marketing communications is not a condition of receiving assistance.

8. Privacy and Information Sharing

Your use of the website is subject to our Privacy Policy (linked in the footer) and any applicable state notices, which describe what information we collect, how we use it, and the authorized organizations with whom it may be shared for eligibility review and service coordination. We do not sell personal information or use it for unrelated marketing. Where we handle protected health information on behalf of a HIPAA covered entity, we do so only under a written Business Associate Agreement.

These Terms are governed by the laws of the State of Utah, without regard to conflict-of-laws principles, except where the mandatory consumer-protection or Medicaid laws of your state of residence apply.
