1. Overview

This Privacy Policy explains how Vista Verde Homes ("we," "us," "our"), through its Medicaid Meals program, collects, uses, shares, and protects information. A core principle of this policy is a data wall: information you provide for Medicaid eligibility screening is used only for education, screening, and referral coordination, and is never used for, or shared for, advertising or affiliate marketing. We do not sell personal information.

2. Information We Collect

Contact information — name, email, phone, mailing address.

Location information — ZIP code, city, state.

Demographic/household information relevant to program eligibility.

Screening responses — answers to social-needs and nutrition-related questions ("Medicaid screening data").

Medicaid coverage information, including your Medicaid ID (CIN), which our eligibility form requires so the plan or Social Care Network we refer you to can find your record.

Technical/usage data — IP address, device/browser type, pages visited, via cookies and similar technologies.

3. How We Use Information

We use Medicaid screening data only to provide education, conduct non-clinical eligibility pre-screening, identify and present available resources, coordinate and make referrals to authorized organizations, communicate with you (subject to consent where required), maintain records for program integrity and audit readiness, and comply with law.

4. The Data Wall (Medicaid Screening Data)

Medicaid screening data and any protected health information are not used for advertising or affiliate targeting, are not shared with advertising or affiliate partners, and are not sold. This data is used solely for the navigation and referral purposes described in Section 3.

5. Advertising and Affiliate Activity (Non-Reimbursable Offerings)

Our website may display advertising and affiliate content for products and services that are not reimbursable by any federal health care program. This activity may involve cookies, analytics, or advertising technologies that collect technical/usage data (Section 2) for measurement and (where applicable) advertising purposes. This is kept separate from your Medicaid screening data per the data wall. See our Advertising & Affiliate Disclosure, and our State Privacy for opt-out rights where applicable.

6. How We Share Information

We share Medicaid screening data only with authorized organizations involved in eligibility review and service coordination (healthcare providers, Medicaid Managed Care Plans, Social Care Networks, community-based organizations, nutrition/meal providers, care navigators) for those purposes, and with service providers acting on our behalf under confidentiality obligations, and as required by law. We do not accept payment in exchange for referrals of federally reimbursable services, and we do not share Medicaid screening data with advertising/affiliate partners.

7. De-Identified and Aggregate Data

We may create and use de-identified, aggregate data (which does not identify any individual) for research, reporting, program improvement, and bona fide fixed-fee service contracts. De-identification is performed so that data cannot reasonably be re-identified, and we do not attempt to re-identify it.

8. Cookies, Analytics, and Your Choices

We use cookies and similar technologies to operate the website, understand usage, and support the advertising/affiliate activity described in Section 5. You can control cookies through your browser, and we provide opt-out choices as described in our policies.

9. Data Retention

We retain information only as long as necessary for the purposes here, for program-integrity/audit needs, or as required by law, then securely delete or de-identify it.

10. Data Security

We use reasonable administrative, technical, and physical safeguards. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.

11. Your Rights

Depending on your state, you may have rights to access, correct, delete, or limit use of your information and to opt out of certain processing. See our State Privacy Laws. Contact info@medicaidmeals.org. We will not discriminate against you for exercising privacy rights.

12. Health Information and HIPAA

Vista Verde Homes is generally not a HIPAA covered entity. Where we handle protected health information on behalf of a covered entity, we do so only under a written Business Associate Agreement. See our HIPAA Disclaimer.

13. Children's Privacy

The website is intended for adults; we do not knowingly collect information from children under 13.

14. Third-Party Websites

We are not responsible for the privacy practices of third-party organizations or advertisers we link to; review their policies directly.

15. Changes

We may update this Policy; the current version will be posted with its effective date.
