What is Medicaid Meals?
Food as medicine
Medicaid Meals is a nonprofit outreach program of Vista Verde Homes, a 501(c)(3) (EIN 99-1152633). We help Medicaid beneficiaries find and connect with medically tailored meal (MTM) and home-delivered meal programs as part of their health care and recovery.
Our eligibility navigation is free for individuals. We are a neutral connector: we do not provide meals, decide eligibility, or enroll anyone. We help you understand and reach the programs you may qualify for, and you choose among the providers available through your plan or Social Care Network.
We present options neutrally and receive no compensation tied to the volume or value of referrals. Where a Medicaid plan or Social Care Network contracts with us for outreach, education, or navigation, we are paid a fixed fee for that work.
What we do

Who may qualify
Each program decides who qualifies. Medicaid nutrition programs generally look for people who:
- Have Medicaid (in New York, a Medicaid Managed Care plan)
- Have trouble affording, shopping for, or preparing food
- Are managing a chronic condition such as diabetes, heart disease, or kidney disease
- Are recovering from an illness, injury, or hospital stay
- Are pregnant or recently gave birth
- Have an intellectual or developmental disability, a serious mental illness, or a substance use disorder
Final eligibility is decided by the program based on your coverage, medical need, and program rules. Participation is voluntary.
Why Medicaid Meals exists
Millions of Medicaid members, especially in rural and underserved communities, struggle with food insecurity and with programs that are hard to navigate. Those gaps lead to preventable health problems, hospital readmissions, and lost independence. We treat food as part of health care and help people reach the nutrition that supports healing and stability at home.

Vista Verde Homes works to close gaps in access to housing, food, health care support, and internet connectivity for rural, underserved, senior, and low-income communities.
New York eligibility
In New York we ask a few questions and, with your consent, connect you to your Social Care Network for the official screening. Your answers may be shared with health care providers, Medicaid health plans, and Social Care Networks to see which services you qualify for. Participation is voluntary.
New York adds its own rules: enhanced nutrition services require Medicaid Managed Care enrollment, membership in one of the State’s enhanced populations, and an unmet need identified through the standardized State HRSN assessment. Services are time-limited: generally up to six months at a time, renewable once if the need continues, and longer during pregnancy and the year after birth. Members in fee-for-service Medicaid receive screening and navigation.
